Fremont, CA: Force Therapeutics, the leading patient engagement platform, and Xealth, the leader in enabling digital health at scale, established a partnership that will simplify clinicians to prescribe and monitor remote care management from within their existing workflows. The Force platform provides provider-prescribed digital education and video content for patients recovering at home from injury or surgery. In addition, the collaboration integrates the Force platform with a provider's electronic health record (EHR) system, allowing care teams to view clinically relevant metrics within the patient's chart and make more informed recovery decisions. "We are pleased to collaborate with Force Therapeutics to make it easier for clinicians to incorporate digital resources into treatment plans, monitor patient activity between appointments, and determine whether patients are progressing as expected." The integration improves physicians' workflow by directly monitoring postoperative data and remote behavior from the EHR. Event-based triggers, such as when a patient is scheduled for surgery, can be used by the integrated platforms to automatically prescribe the Force platform without the need for physician intervention. Care teams can track patient data such as reported daily pain, medication usage, activity levels, platform engagement, patient-reported outcomes, and physical therapy records from within the patient's chart. The collaboration enables clinicians to practice effective care management at scale by incorporating digital health into provider workflows. Xealth allows doctors and patients to access relevant, timely, efficient, and targeted digital apps, devices, and services, assisting hospitals and health systems tightening patient connections. Xealth is currently compatible with all Epic and Cerner EHR systems, which account for 56 percent of the total EHR market share. In addition, because Xealth handles much of the development and implementation work, the platform requires few IT resources to maintain.
